What All-on-6 is — and when six implants beat four

If you've researched full-arch implant restoration, you've almost certainly seen the term "All-on-4" treated as a default. It's the most heavily marketed protocol in modern dentistry. But for a meaningful share of patients, particularly those with higher bite forces or those restoring the upper jaw, four implants aren't the right number. Six are.

All-on-6 follows the same core principle as All-on-4: a fixed bridge of teeth supported by dental implants instead of resting on the gums like a denture. The difference is the foundation. Six implants distribute bite force across more anchor points, reduce stress on any individual implant, and provide a more conservative long-term mechanical reserve — meaning if one implant ever has a problem, the bridge is still securely supported by five.

For our patients, the choice between All-on-4 and All-on-6 is made during the 3D cone-beam CT consultation, not from a marketing brochure. We measure bone density, evaluate bite force, examine opposing dentition, and recommend the protocol that gives you the best long-term outcome — not the one with the lowest sticker price.

All-on-6 vs. All-on-4: when six implants are the right call

The right number of implants depends on your jaw, your bite, and your goals — not on marketing. Here's how the two protocols compare across the clinical factors that actually drive the decision.

Clinical factor All-on-4 fits All-on-6 fits
Jaw being treatedLower jaw (mandible) — denser boneUpper jaw (maxilla) — softer bone
Bite force / clenchingNormal biteHeavy chewers, bruxism, history of grinding
Bone densityAdequate to strongSofter bone where extra anchorage helps
Opposing teethDenture or other implant archNatural teeth or dense crown work creating high force
Long-term redundancyAcceptablePatient prioritizes mechanical reserve
Cost sensitivityHigher (lowest implant count)Slightly higher initial investment

What this looks like in real cases

Most upper-arch full-restoration patients in our practice end up with All-on-6 rather than All-on-4. The upper jaw bone is softer than the lower jaw, the patient often has natural teeth on the bottom (creating high opposing bite force), and the additional implants meaningfully improve long-term stability.

For lower-arch cases, All-on-4 is more often appropriate — the mandibular bone is denser, and the four-implant protocol has decades of documented success there.

When patients ask us which is "better," the honest answer is: All-on-6 is generally more conservative and provides more mechanical safety margin, while All-on-4 is more cost-efficient when the anatomy supports it. The right answer for you depends on your specific bone, bite, and goals.

What the All-on-6 procedure looks like

A predictable, four-stage protocol — from your first 3D scan through your permanent zirconia bridge.

Step 1 — Consultation and 3D imaging (Day 1)

Free consultation with Dr. Huang. Cone-beam CT scan that lets us measure bone height, width, and density at every potential implant site. Treatment plan with itemized pricing, written and given to you to take home.

Step 2 — Surgical planning (Days 2–14)

Using your CT data, we design a surgical guide that determines exactly where each of the six implants will be placed — angles, depths, and positions optimized to your specific anatomy. Your custom temporary bridge is fabricated before surgery.

Step 3 — Surgery day

Under IV sedation administered by a board-certified anesthesia provider, failing teeth are extracted (if applicable) and six implants are placed using the surgical guide. The fixed temporary bridge is attached the same day. You walk out with teeth.

Step 4 — Healing and final bridge (3–6 months)

The implants integrate with your bone over 3–6 months. During this time, you wear the temporary bridge and eat soft to medium-texture foods. After full integration, your final custom zirconia bridge is attached — your permanent teeth.

What's included in our $19,995 All-on-6 fee

We publish prices because the alternative — "starting at" pricing with major costs disclosed only after the consultation — is the single biggest source of frustration patients describe when they come to us for second opinions.

Our All-on-6 zirconia fee includes:

  • Pre-surgical 3D cone-beam CT scan
  • Surgical planning and guide fabrication
  • Extractions of failing teeth (full arch)
  • Six premium dental implants
  • IV sedation (board-certified anesthesia)
  • Same-day fixed temporary prosthesis
  • Custom prototype try-in appointment
  • Final custom zirconia bridge
  • Post-operative care visits (first year)
  • 10-year material warranty

Total per arch: $19,995
Or financed: From $389/month with approved credit

If your case requires bone grafting, sinus lifts, or other ancillary procedures, those are quoted separately during your consultation — never as a surprise after surgery.

Why patients across Las Vegas choose Significance for All-on-6

All-on-6 is one of the most demanding protocols in implant dentistry. The single biggest predictor of success is the surgeon's experience — here's what sets our team apart.

Specialist surgical training, not weekend coursework — All-on-6 performed by a board-certified periodontist with formal residency in implant placement and bone regeneration.
Volume that matters — our team has placed over 10,000 dental implants. Surgical fluency separates predictable outcomes from complications.
Complex cases welcome — failed prior implants, bone loss, complex medical histories. If All-on-6 isn't right for your case, we'll tell you.
Transparent pricing, written treatment plans — every patient leaves the consultation with a written, itemized plan. No add-on fees disclosed mid-treatment.
Personal post-operative care — Dr. Huang personally calls every patient on the evening of surgery. You'll have his direct number for the first 72 hours.
Four locations across the Las Vegas valley — Harmon Avenue, Cascade Valley, Fort Apache Road. Your records and care team move with you.

What patients ask before All-on-6

The number refers to how many implants support the full-arch bridge. All-on-6 uses six implants instead of four, distributing bite force across more anchor points and providing mechanical redundancy. For the upper jaw, where bone is softer and forces are higher, All-on-6 is often the more conservative choice. The right number for you is determined by 3D imaging and bite analysis during your consultation.
Our All-on-6 zirconia fee starts at $19,995 per arch — or as low as $389/month with approved financing. That includes the implants, IV sedation, the same-day temporary, and the final zirconia bridge. Most other Las Vegas practices range from $24,000 to $40,000+ for comparable treatment.
Surgery itself takes 3–5 hours per arch under IV sedation. Total time at our office on surgery day, from arrival to discharge, is typically 6–7 hours.
Yes. The All-on-6 protocol — like All-on-4 — is designed for immediate loading. A fixed temporary bridge is attached to the implants the same surgical day.
The titanium implants are designed to last a lifetime when properly maintained. The temporary acrylic bridge typically lasts 1–2 years before being replaced with the final zirconia bridge, which lasts 15–25+ years.
No. Once integrated and the final bridge is in place, patients cannot tell whether their bridge is supported by four, six, or eight implants. The functional difference is in the long-term stability and mechanical reserve, not in day-to-day feel.
No. For the lower jaw with adequate bone density and normal bite force, All-on-4 has decades of documented success and is often the more efficient choice. All-on-6 wins specifically when bone is softer (typically upper jaw), bite forces are higher, or the patient prioritizes mechanical redundancy.
Most dental plans cover a portion — particularly for the extractions and the prosthetic components. We verify your specific benefits before treatment and credit any insurance reimbursement directly to your account.
Mild to moderate bone loss is often manageable with All-on-6 plus minor grafting. Severe upper-jaw bone loss may indicate that zygomatic implants are a better fit — anchoring into the cheekbone and bypassing the jaw entirely. The 3D scan during consultation determines which option fits your anatomy.
In some cases, yes — additional implants can be added later to convert an All-on-4 to an All-on-6. This is more commonly considered when the original All-on-4 has shown stability concerns. We evaluate these cases individually.
